Simple answer. Infrastructure and the civic bodies they deal with. Karachi for example. Easier to deal with DHA as opposed to KDA or KMC. The infrastructure is bad in older areas. They don't know where they can lay cables or not and no master plan exists. In other city areas often the apartments have their own bodies that create hurdles. Cable theft and damage to underground cables is a bigger threat in older areas where vigilance is sparse. These are some basic reasons.

The main thing is, if a company cannot provide services in some areas, they didn't have the right to take money from the residents and make false promises. And even if you did take the money and after uncontrollable circumstances you should morally return the money. We 2 people are waiting for services for the past 2.25 years to get the connection, after being falsely promised by one such company.
